Millions of children all over the globe benefit from antibiotics each year. They’re among the most precious tools modern medicine has, credited with saving countless lives. Yet, here’s the uncomfortable truth: these medicines are becoming less effective year after year, and experts predict the situation will only worsen over the next ten years. If nothing changes, this could spell a dramatic rise in child mortality across the world, as revealed by Australian researchers in a study published in JAMA Pediatrics.
The culprit? Antimicrobial resistance (AMR)—in everyday language, bacteria learning how to dodge and survive the very antibiotics designed to wipe them out. As if that’s not scary enough, this resistance is spreading faster than expected, leaving health professionals with fewer options for treating even the simplest infections.
Antibiotic resistance isn’t just a hospital problem or a health scare making headlines. It’s a phenomenon that’s becoming alarmingly widespread—including right in the air we breathe.
To highlight just how widespread and urgent the issue is, scientists at the Murdoch Children’s Research Institute (MCRI) and the University of Sydney analyzed bacteria from 106,581 infection samples collected between 2004 and 2022 from children aged 0 to 18 in 82 different countries. Their conclusion:
“Between 2004 and 2022, pediatric antimicrobial resistance increased in all regions, with consistently higher resistance levels and faster progression in resource-limited settings,”
they wrote in their article.
Not all bacteria are equally villainous, but some are especially worrisome. Acinetobacter baumannii, responsible for a range of infections from the lungs to the skin—particularly in hospital settings—showed the highest resistance rates. The study found that 55% of the Acinetobacter baumannii samples from children were resistant to several antibiotics.
Then there’s Klebsiella species, which can cause everything from urinary tract infections and hospital-acquired illnesses to sepsis. These bacteria showed “the fastest rate of progression, particularly in relation to third- or fourth-generation cephalosporins and carbapenems in Southeast Asia, Eastern Europe, and the Western Pacific region.”
The World Health Organization (WHO) has raised repeated alarms over these bacteria that are becoming nearly impossible to kill. As they resist treatment and spread beneath the radar, they threaten to make our arsenal of antibiotics useless. According to WHO, some infections could soon become fatal again—and honestly, nobody is prepared for this comeback.
Penelope Bryant, co-author of the study and associate professor at MCRI, told The Guardian: “We are seeing antibiotic resistance not only in severe diseases but also in simple, uncomplicated infections like urinary tract infections in children.”
Antimicrobial resistance is a major public health issue. But why is it hitting children so hard?
According to the authors, children have been widely neglected when it comes to monitoring and research on antimicrobial resistance. Clear guidelines for children’s dosing are rare, and there’s a marked reluctance to conduct clinical trials involving children. What’s more, developing pediatric antibiotic treatments is simply less profitable for pharmaceutical companies—hardly a motivation for rapid progress.
Let’s not forget the environmental dimension. A familiar campaign slogan in French media once reminded everyone: “Antibiotics, it’s not automatic!” Overusing antibiotics is not only bad for our health, but also disastrous for the environment—affecting our water, soil, plants, and animals. And, as always, what damages the environment eventually circles back to impact human health.
Why is the situation particularly dire in low-income countries? There, antibiotics are often poorly regulated, and sanitation can be inadequate. Resistant infections are now among the leading causes of death in children under five in these regions.
In wealthier countries, overconsumption of antibiotics in agriculture, animal husbandry, veterinary care, and human health services is the main culprit. However, in these settings, failed antibiotic treatments for children with resistant infections remain much less common—at least for now.
Bottom line? The miracle of antibiotics is fading fast—and without urgent international cooperation, research, and sensible use, the next generation may find themselves fighting diseases their parents considered beatable decades ago.
